چکیده

Objective: Transfusion-related acute lung injury (TRALI) is a relatively uncommon complication in patients who undergo plasma-containing blood product transfusion. Despite the cross matching process, TRALI remains the most common cause of mortality associated with transfusion. We describe a case of TRALI in a 24year-old trauma patient who was successfully cared for on extracorporeal membrane oxygenation (ECMO). Design: Chart review. Setting: Academic medical center with Level 1 Trauma. Patients and participants: Single case from a busy urban trauma center. Interventions: Extracorporeal membrane oxygenation. Conclusion: ECMO has only rarely been successfully used in patients with TRALI; to our knowledge, ours is the first successful report of its use in a trauma patient. Its use in maintaining gas exchange in trauma patients with TRALI should be further investigated. Suspected Transfusion Related Acute Lung Injury Improving following Administration of Tranexamic Acid: A Case Report

A 16-year-old woman with craniofacial injury developed severe acute respiratory failure under the primary reconstructive surgical procedure requiring several units of blood and plasma. A transfusion related acute lung injury (TRALI) was suspected and supportive treatment was initiated. Because of the severity of symptoms, acute extracorporeal membrane oxygenation (ECMO) was planned. During prep...

Successful treatment of a severely injured soldier from Afghanistan with pumpless extracorporeal lung assist and neurally adjusted ventilatory support

BACKGROUND Life-threatening acute lung injury due to combat and/or terror attacks is associated with high mortality. The successful management includes the use of "rescue" extracorporeal lung assist and early transport by aeromedical evacuation teams. AIMS Description of the pre-hospital support of a severely injured soldier with a pumpless extracorporeal arterio-venous lung assist in critica...

Extracorporeal membrane oxygenation with double venous drainage.

We report on a successful configuration strategy of extracorporeal membrane oxygenation(ECMO) in two consecutive cases of acute lung injury. A 60-year-old woman with Streptococcus pneumoniae infection and a 22-year-old man with hemothorax were admitted to our hospital with failing lungs. Although treatment with a ventilator was started, oxygenation could not be maintained. ECMO with a femoro-fe...

Severe hypoxemia during veno-venous extracorporeal membrane oxygenation: exploring the limits of extracorporeal respiratory support

OBJECTIVE Veno-venous extracorporeal oxygenation for respiratory support has emerged as a rescue alternative for patients with hypoxemia. However, in some patients with more severe lung injury, extracorporeal support fails to restore arterial oxygenation. Based on four clinical vignettes, the aims of this article were to describe the pathophysiology of this concerning problem and to discuss pos...
